The access you need, when you need it!

New West Physicians is excited to offer our patients the latest in healthcare technology and personal healthcare management!

Our new “My Health Connection” portal, gives you secure and encrypted access to your medical record 24/7, providing you with a convenient way to review and manage your health care record, and to communicate with your doctor’s office online. This service is available to all New West patients at NO CHARGE.

 

 

 

 

 

 


My Health Connection is your own Personal Health Record ("PHR") with additional health information being provided from your New West Physicians provider. Your PHR is maintained by you and can include any information that you feel affects your health. Examples include tracking your weight and blood pressure.

IMPORTANT:

The information that you choose to add WILL NOT alter your New West medical record, and not all information that you put into your PHR will automatically be reviewed by your provider.


It is also important to note that My Health Connection should NEVER be used to communicate urgent situations in which you feel there is a health emergency. In these situations, PLEASE call 911.

Your provider's office will answer messages and requests from My Health Connection within 1-2 business days. If you have an issue that is more urgent, please call the office.

How do I sign up?

Option #1

At your clininc, the front desk will have sign up brochures for MyHealthConnection.
Complete the sign up portion and return it to the front desk staff. They will then send you an invitation via email. Complete the invitation process (shown below) and your connection will be started!


Option #2

Go to the My Health Connection login page (HERE) and follow the instructions to request an
account. Once you have completed the request, your account will be created in 5 business
days by our My Health Connection Team!

  • You must have a Facebook, Google, Yahoo or Windows Live account to request an account on MyHealthConnection. If you do not have one of these, you will be given the option to create a free account.

    • Message request for a call back or follow up – 1 business day
    • Appointment request – 1 business day
